Provides or collaborates with IDT to provide care needed to address patient/family immediate needs, as identified in the initial assessment. Conducts and documents start of/completion of comprehensive assessments accurately and within required time frames for all assigned patients. Coordinates/communicates necessary information with other members of the IDT, consulting physician(s), facility staff, and others as appropriate.
