Job Summary The Utilization Management Nurse Specialist RN II facilitates, coordinates, and approves medically necessary referrals that meet established criteria. Assures timely and accurate determination and notification of referrals and reconsiderations based on the referral determination status. Generates approval, modification and denial communications, to include member and provider notification of referral determination. Actively monitors for admissions in any inpatient setting. Performs telephonic and/or onsite admission and concurrent review, and collaborates with onsite staff, physicians, providers, member/family interaction to develop and implement a successful discharge plan. Works with the UM Manager and Physician Advisor on case reviews for pre-service, concurrent, post-service and retrospective claims medical review. Monitors and oversees the collection and transfer of data (medical records) and referral requests by Providers. Acts as a department resource for medical service requests /referral management and processes. Receives incoming calls from providers, professionally handles complex calls, researches to identify timely and accurate resolution steps. Follows up with caller to provide response or resolution steps. Answers all inquiries in a professional and courteous manner. Duties Promote and support team engagements, programs and activities to create and ensure a positive and productive workplace environment. Perform telephonic and/or onsite admission and concurrent review, and collaborates with onsite staff, physicians, providers, the member and significant others to develop and implement a successful discharge plan. Process, finalize and facilitate inbound requests that are received from providers. Generate appropriate member and provider communication for all determinations within the required timelines as defined by the most current department policy. Facilitate/review requests for Higher level of care or skilled nursing/discharge planning needs. Research for appropriate facilities, specialty providers and ancillary providers to utilize for all lines of business. Identification of potential areas of improvement within the provider network. Identify and initiate referrals for appropriate members to the various L.A. Care programs/processes and external community based programs or Linked and Carve Out Services (e.g. DDS/CCS/MH). Potential quality of care/potential fraud issues are identified and documented per L.A. Care policy. High risk/high cost cases and reports are maintained and referred to the Physician Advisor/UM Director. Document in platform/system of record. Utilize designated software system to document reviews and/or notes. Receive incoming calls from providers, professionally handle complex calls, research to identify timely and accurate resolution steps. Follow up with caller to provide response or resolution steps. Answer all inquiries in a professional and courteous manner. Perform other duties as assigned. Responsibilities Essential Job Functions : Pre-Procedure Care Perform pre-procedure nursing assessments and review relevant clinical information. Verify patient identity, procedure, NPO status, medication reconciliation and consent according to institutional policy. Initiate intravenous access and prepare patients for procedures. Collaborate with the procedural team regarding case planning. Document required information in the electronic medical record (EMR). Provide patient and family education regarding the procedure and recovery expectations. Intra-Procedure Care The Interventional RN provides continuous patient monitoring and clinical support during procedures and may function in one of the following roles. Sedation RN Administer procedural medications per performing provider order Administer and monitor moderate sedation in accordance with organizational policy and credentialing requirements. Perform a pre-sedation assessment, including verification of allergies and NPO status, and ensure that the airway evaluation and ASA classification are documented in accordance with policy. Ensure laboratory results are within limits for specific procedure, communicating with performing provider results outside of limits and potential need for additional testing. Continuously monitor patient status including cardiac rhythm, blood pressure, respiratory status, oxygen saturation, and level of consciousness. Maintain airway patency and respond to sedation-related complications. Communicate patient status with the procedural team and document medications and monitoring parameters in the EMR. Circulating RN Coordinate nursing care in the procedure room and ensure completion of patient identification, procedure verification, and procedural time-out. Maintain a safe procedural environment and assist with patient positioning and monitoring setup. Assist in administration of medications as needed per performing provider order Assist with vascular access site management when appropriate. Recognize and respond to procedural complications such as bleeding, arrhythmias, or respiratory compromise. Communicate patient status, manage specimens, and document care and procedural events in the EMR. Procedural Monitoring RN Functions as the primary documenter, recording hemodynamic data, pressures, and procedural events. Ensures allergy considerations are communicated during the timeout and reflected in procedural documentation. Operates and troubleshoots physiological monitoring systems (e.g., hemodynamic recorders, ECG, oxygen saturation). Documents all supplies used, implants, contrast usage, and procedural details accurately. Post-Procedure Care Receive and recover patients following procedures and perform post-procedure assessments including vital signs, neurological status, and access site evaluation. Maintain airway patency and monitor for hemodynamic or respiratory changes. Manage pain, nausea, and other recovery needs. Assess and manage vascular access sites and maintain hemostasis. Administer medications and treatments as ordered. Educate patients and caregivers regarding recovery, medications, activity restrictions, and signs of complications. Coordinate patient transfer or discharge and provide appropriate handoff communication. This 28 private-room unit, with the capacity to care for up to 44 patients and an average daily census of 17, supports neonates ranging from 22 weeks gestation through full term with a wide variety of complex conditions.
