Our National After-Hours Call Team plays a vital role by providing compassionate telephonic care and clinical direction during evenings, nights, weekends, and holidays—ensuring that residents receive timely, high-quality support without unnecessary transfers. In this work-from-home role, you’ll deliver after-hours care virtually (primarily by phone) to aging residents across multiple states. This position offers both autonomy and purpose—you’ll be the trusted voice and clinical partner helping residents and facility staff during critical times, making an immediate impact in the lives of older adults. Provider must be comfortable managing high call volumes and performing at least 30% telehealth visits, including evaluation of acute changes, falls, and controlled substance visits. Providers must be able to manage multiple calls independently while providing care across several states. Essential Duties & Responsibilities Serve as the first line of support for residents and facility staff after-hours, providing direction and medical care over the phone. Use Curana’s telephonic platform to take and place calls, coordinating care between facilities, hospitals, and clinics. Deliver high-quality, cost-effective care to patients—addressing acute, chronic, and behavioral health needs in collaboration with physicians and specialty providers. Perform comprehensive assessments and document encounters accurately and thoroughly in the EMR, ensuring compliance with CMS requirements. Apply Curana’s clinical protocols and practice guidelines to support safe, effective treatment in place whenever possible. Participate in mandatory education and training to stay current with standards of care. Scheduling & Hours: While shift times can vary, we provide coverage to skilled nursing and senior living facilities on weeknights from 5pm- 8am local time, continuous coverage from Friday at 5pm to Monday at 8am. Holiday coverage is also provided beginning at 5pm of the end of the last business day to 8am of the resumption of business hours. Availability and Coverage expectations for this role Weeknight shifts between 5pm and 8am Every other weekend coverage for 12-hour shifts covering night shifts 6pm-6pm on both Saturday and Sunday Overnight and holidays are required for all After Hours Call Team Members, 2 holidays required per year. Holiday scheduling is completed at the beginning of the year for advanced planning Qualifications Education and Experience: Master's Degree as a Nurse Practitioner Active licensure in North Carolina and South Carolina required. Additional active licenses in Florida and Georgia are strongly preferred. Candidates must be able be willing to obtain additional state licenses as needed within a 30-day timeframe. Our Medical ICU clinical team has expertise in the management of many Critically ill patients including: Respiratory Failure: ARDS, COPD, Asthma Shock: Septic shock, hypovolemic shock, cardiogenic shock Hepatic failure – GI bleed Renal failure and metabolic altered mental status DKA MICU Primary Team / House Staff Provide direct management of critically ill patients in the MICU Perform admissions, daily management, and coordination of care Collaborate closely with attending physicians, fellows, and multidisciplinary teams Participating in rounds, handoffs, and escalation of care Expanded MICU Coverage (6 Additional Beds) Provide continuous 24/7 coverage for an additional MICU service Function with a mix of autonomy and collaboration depending on census and acuity Ensure continuity of care across shifts with structured handoffs Procedure Service Rotate through and perform bedside procedures including lumbar puncture, PICC placement, paracentesis, and thoracentesis Maintain procedural competency and volume expectations Oncology Critical Care Consults Respond to and manage ICU-level consults for oncology patients Partner with oncology and ICU teams for escalation and co-management Schedule & Expectations Rotating day and night shifts (24/7 model) Weekends and holidays required Shared accountability for coverage across all service lines Expected flexibility based on census and operational needs Nurse Practitioner Job Summary: The Nurse Practitioner is an advanced practice nurse responsible for comprehensive direct patient care management. The Nurse Practitioner manages a caseload of patients and exercises autonomous decision making in the assessment, diagnosis, and initiation of delegated medical treatments. The scope of practice also includes family support, teaching, staff education, consultation, and research. Advanced Practice Nurse: Provide comprehensive care to patients in the Medical Intensive Care Unit or inpatient units cared for by the Medical ICU service at the University of Chicago. Work collaboratively within a multidisciplinary team, using advanced clinical skills to manage a caseload of patients. Assess, diagnose, initiate delegated medical treatments, perform independent nursing interventions, and evaluate care. Provide a model of care that is shaped by theory, expanded by science, research and reflective practice and is identified by a holistic and comprehensive approach to the patients. Nurse Practitioner - Education and Experience Requirements: Advanced degree (or equivalent, if license received prior to July 1, 2001) from an accredited school/college of nursing required. Ability to obtain Illinois advanced practice nurse (APN) license by start date required. Current license as a Registered Nurse required. AANC certification as an ACNP and/or ANP-BC required. Minimum two years of critical care clinical nursing experience with demonstrated clinical competence in patient care, teaching and management of patients with critical illness, and knowledge about clinical aspects of critical care including protocol therapy preferred. Physician Assistant Required Qualifications Completion of an accredited Physician Assistant Master’s program Current and/or eligibility for State of Illinois PA licensure / registration Must have or be able to qualify for Illinois and DEA prescribing numbers Must have AGACNP certification but FNP or ANP will be considered with relevant experience as a PA or NP.
