Nurse Practitioners practice in our outpatient practices on a collaborative care team composed of a physician, nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. As such, our healthcare providers can focus on care within the clinic: primary care, care coordination, and population health. Core Responsibilities: Provision of exceptional primary care. Conduct office visits for routine and acute issues. Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation. Care coordination with other providers, specialists, testing facilities, and agencies. Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years) Assisting the care team with phone triage and outreach. Educating patients on their health conditions, care plans, and treatments. Participating in Oak Street Health promotional activities. Conducting home visits as needed. Other duties, as assigned. May perform those routine medical treatments and diagnostic procedures for which they are qualified to perform by training and which fall within the normal scope of practice of the supervising physician. • Performance of histories and physical exams, pre and postoperative patient care, critical care, daily progress notes, obtain surgical consent, patient education, and discharge summaries. • Patient care in ICU including advanced knowledge of hemodynamic management, ventilators, and artificial cardiac devices. • First assist in surgery in all cardiac, thoracic and vascular procedures. Recognize serious, life-threatening emergencies, evaluate them appropriately and ensure patient receives prompt care to stabilize. Triages patient calls and evaluates patient problems. • Opening and closing of sternotomies, emergency opening of post op cardiac patient, emergency intubation, perform required would care under the direction of the CT Attending. • Educator and trainer for nurses, residents and fellows. • Participates in quality management and performance improvement activities. • Call coverage may be required specific to departmental needs. Order and interpret appropriate diagnostic tests, such as routine laboratory, X-ray, Ultrasound etc. Monitors the patient’s course of treatment and implement care plans. Participate in daily inpatient rounds. Evaluate consultations in the emergency department and outpatient office. With appropriate training and supervision, perform bedside procedures such as newborn circumcision, extra digit excision, drainage of abscess, local wound care, ostomy and gastrostomy tube care. The hired APRN will care for critically ill patients, pre and post operatively, with congenital heart disease. They will often manage patients on vasoactive support, mechanical ventilation, CRRT, and ECMO or Ventricular Assist Devices. Patient population varies from neonates to adults with congenital heart disease that require repeat intervention. The hours for this position will include three thirteen hours shifts including rotating days/nights/weekends and holidays. Call will also be required. Previous APP experience is preferred, however new graduates with ICU experience are welcome to apply. Job Responsibilities: Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of illness, and scrutinizes lab data on new and former clinic patients and hospital admissions to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan.
