Position Summary: The primary function of the Assistant Director of Nursing is to assist the Director of Nursing in planning, organizing, developing, managing, and directing the day-to-day functions of the SNF Nursing Service Department in accordance with current federal, state and local standards, guidelines and regulations that govern the facility. The Assistant Director of Nursing will work alongside the Director of Nursing with the Administrator and the Medical Director to ensure the highest degree of quality of care is maintained for each resident at all times. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the facility's administration and Nursing Policies and Procedures. Education/Qualifications: Must possess a current, unencumbered license to practice as a RN within the state in which you work or licensed as an LPN if the state you operate in allows. Minimum two (2) years of experience in a supervisory role in a hospital, nursing facility or other related healthcare facility, preferably in long term care. Must possess basic computer skills and ability to navigate through various software programs, including electronic health record software. Job Responsibilities: Completes assessments as established by unit standards and addresses physical, social, cultural, spiritual and psychological needs of patient and family. Formulates and documents plan of care as established in unit standards. Includes health teaching, family needs and discharge planning. Administers medications and treatments according to procedure. Evaluates effectiveness of nursing interventions and revises plan of care based upon patient response to interventions or lack of progress. Coordinates and provides patient care, discharge planning and patient and family education. Demonstrates skills and knowledge necessary to assure assessment, treatment, and care is appropriate to the age of patients served. Initiates collaboration with physicians, ancillary personnel, and other interdisciplinary team members and acts to proactively resolve and facilitate action to meet patient needs. Delegates on the basis of skill, expertise and time availability of the team member, acuity and needs of the patient. Problem solves, troubleshoots, and takes corrective measures in care and handling of equipment. Actively participates as a team member working toward team goals of the organization. Actively participates in education and professional development activities. Assumes responsibility for identifying processes or systems that could potentially lead to errors and adverse events. Participates in problem solving when appropriate. Actively participates in patient safety improvement and management program. Responds to all Adult Rapid Responses and Adult Codes throughout Geisinger Medical Center. This includes but not limited to Sepsis Alerts, Pulmonary Embolism Response Team calls, and Stroke Alerts. Assists in initial resuscitation and implementation of bundle guidelines in accordance with best practice. Assists in the determination and transportation of the patient to the appropriate level of care. Completes documentation of events during Rapid Response calls in the medical record. Maintains and updates Rapid Response Database. Performs proactive rounding in patient care areas other than intensive care in order to identify changes in patients ’conditions and effect early intervention in collaboration with outreach team or nocturnist. Under general supervision, provides direct patient care to assigned patients. Education: Associate Degree from an accredited School of Nursing required. Experience: Two year Registered Nurse experience required in the specific specialty area o r LPN experience at GBMC with director approval. Licensures/Certifications: Current registration with the Maryland State Board of Examiners of Nurses as a Registered Nurse. Current Healthcare Provider BLS certification from an affiliated American Heart Association training site Additional Certifications (i.e. ALS, PALS, NRP) are required upon hire for specialty areas Skills: • Proficiency in developing nursing plan of care, implementing effective nursing care, and evaluating outcome of nursing intervention. • Excellent communication and interpersonal skills. • Effective planning and organizational skills in order to effectively manage multiple priorities simultaneously • Strong analytical skills • Effective skills in use of computer programs • Demonstrates skill in patient experience and service recovery Principal Duties and Responsibilities: Utilization of Nursing Process A. Assessment. • Compiles database and formulates nursing diagnosis. • Utilizes physical assessment and interviewing techniques. • Anticipates need for intervention by other disciplines. • Determines priorities for patient care needs or problems. • Reassesses the patient as indicated by changes in condition and/ or response to treatment(s) B. Planning. • Initiates and updates a written nursing plan of care for patients by incorporating scientific and psychosocial principles that support the nursing diagnosis. • Incorporates information obtained from nursing care planning conferences into written nursing plan of care. • Coordinates patient, family, significant other, and health team members in the planning of hospital and post-discharge care. C. Implementation. • Performs clinical nursing skills independently. • Administers medications, IV therapy and treatments appropriately and accurately. • Initiates appropriate emergency procedures as necessary. • Appropriately delegates and coordinates the duties of ancillary personnel. • Communicates the patient's status and plan of care through written documentation in the patient's medical record. • Verbally communicates the patient's status to physicians, nurses and other health care professionals. • Communicates in a therapeutic manner with the patient, family and visitors. • Applies teaching/learning principles in education of patient and family. D. Patient Care Coordination • Manages daily patient assignment by appropriately setting priorities. • Delegates responsibility appropriately and monitors the nursing care delivered to patients by ancillary personnel. • Coordinates patient activities with other disciplines. E. Teaching and Quality Improvement • Develops, documents and implements Patient Teaching and Discharge Planning Record. • Participates in established patient, family, staff and/or student education program. • Participates in clinical monitoring at the unit level. Identifies unit, staff and patient care problems. • T akes action or refers problems to appropriate resources when indicated. F. Professional Excellence Model • May participate in activities of the RN 3 with the exception of serving as a facilitator for the Nurse Residency Program • If competent, serve as a charge RN and monitor productivity/patient flow • If competent, precept new staff Physical Requirements Ability to stand and walk almost constantly up to 90% of work time and lifting and positioning patients up to 50% of work time.
