Location: Main Hosp Bldg - 4th Floor OPC Schedule: Nights, 7p - 7a PRIMARY PURPOSE The RN is a licensed professional who uses the Parkland nursing professional practice model to coordinate patient care delivery, that focuses on a particular patient population(s)/specific health conditions, by the health care team. Using the nursing process, the RN assesses the patient, identifies nursing diagnoses based on responses to health problems, develops and implements an individualized plan of care, and evaluates the patient's response. The RN utilizes knowledge of patient needs and the healthcare environment to assist patients to transition through the healthcare encounter without any preventable complications or delays. The RN delegates interventions to health care personnel based on the Texas Nursing Practice Act, each patient's condition and the competencies of the employee. MINIMUM SPECIFICATION Education Must be a graduate from an accredited school of nursing. Experience Must have two years of nursing experience in a level III or higher Neonatal Intensive Care Unit. Prefer experience in an acute care setting. Equivalent Education and/or Experience : None Certification/Registration/Licensure Must have current RN license or valid temporary permit with the Texas Board of Nursing or a valid Compact RN license. Ensures that all records and documents meet the standards, goals, and objectives of the NICU and Parkland. Works with other staff in providing direction to non-nursing staff. Participates in information exchanges with nursing and other Parkland staff that support the standards, missions, goals and objectives of the NICU and Parkland. Seeks and utilizes direction from more experienced staff and team members as needed. Provides appropriate hand off any time care is relinquished. Serves as Charge Nurse on rotation as necessary. Serves as a preceptor or trainer to newer nursing staff members. Identifies ways to improve work process and improve patient and customer satisfaction utilizing therapeutic communication with family including recommendations for support groups, providing information and resources. Makes recommendations to supervisor, implements and monitors results as appropriate in support of the overall goals of the NICU and Parkland. We place a high importance on our employees and reward staff in several ways such as: • Competitive hospice industry compensation • Benefits package with multiple plan offerings and generous employer contribution • 401(k) Retirement plan with employer match • Supportive work culture which encourages work life balance • Substantial Paid Time Off, paid holidays & birthday benefit • Employee development program managed by in-house education department to support individualized orientation and professional growth needs. • Monthly mileage reimbursement • Employee Referral Program • Business casual attire for office staff Responsibilities JOB DUTIES/KNOWLEDGE/JOB PERFORMANCE: • Develops written plan of care in coordination with an interdisciplinary Team (IDT). • Oversees implementation of plan, evaluates and changes plan as necessary to meet patient/family needs. • Responsible for the direction of care provided by LVN and HA in accordance with patient’s plan of care. • Responsible for the supervision of services provided by the LVN and HA per TEH policy and regulatory requirements. • Communicates all changes to IDT and primary MD. • Performs job duties in clinical areas as stated in clinical competency checklist, policies/procedures, standards of care, and critical competencies based on Scope of Practice. • Maintains timely patient documentation using “point care” electronic charting. • Implements and maintains established hospice policies and procedures as well as Standards of Care and regulatory requirements. • Provides health care instruction to patient and family members. • Orients, instructs, and trains assigned personnel. • Participates in providing 24 hour, seven days a week coverage to patients/families (on-call and census fluctuation). • Performs admission assessment and admission when necessary. DEGREE OF TRAVEL: • Travel required for patient visits. • Occasional travel for educational purposes. Qualifications: • Must have Associates Degree, Bachelor Degree preferred. • Must have one year of acute or hospice pediatric experience • At least 12 months experience with 1-2 years preferred in acute care. • Current CA RN license. • Current CA driver’s license with proof of insurance. • Current CPR/BLS • Prefer certification as hospice nurse. • Pediatric experience preferred. • Proof of recent TB test result • Bilingual preferred. • Computer skills (word processing), use of adding machine, calculator, fax, photocopier.
