The primary purpose is to provide chemotherapy infusion and related treatment. QUALIFICATIONS License /Certification Required: CA RN license BLS Preferred: National certification as Oncology Certified Nurse Education Preferred: BSN, MSN, or currently enrolled in RN-BSN program Experience Required: One year RN experience or LVN IV experience JOB RESPONSIBILITIES Essential Mixes chemotherapy drugs as prescribed by physician after passing competency test. Provides chemotherapy administration in a manner that is consistent with Oncology Nursing Society standards. Provides appropriate functions prior to administration of chemotherapy including consent, insurance approval, and verification of dosage. Accurately provides patient and family education on chemotherapy, side effects, symptom management, and community resources i.e.: American Cancer Society, support groups. Provides appropriate telephone triage for patients and families. Answers telephone in a courteous, professional manner. Accurately inputs patient charges and credits into billing systems as instructed. Inspects and documents medication inventory according to procedures. Maintains competency in the use and care of all venous access devices, central lines. Oversees and directs the work of all staff involved with blood sampling, performing CBC test in addition to performing quality controls for the SYSMEX-KX21. Participates in quality improvement activities as assigned. Documents the orientation and training related to blood sampling and related testing for all staff. Assists in maintaining a clean and orderly work environment. Additional Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area. Knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs. Able to effectively communicate and care for patient and family as reflected in the Plan for Provision of Care. Performs other duties as assigned. CPR certification is recommended (required for direct patient care in Georgia), along with prior experience in hospice, home health, medical-surgical, or population-specific nursing such as geriatrics or pediatrics. Must demonstrate a willingness to participate in an on-call rotation and maintain reliable transportation, valid driver’s license, and state-required liability auto insurance. Strong communication and organizational skills are essential, along with proficiency in Microsoft Office Suite and EMR systems. The ability to collaborate effectively within an interdisciplinary team is required. This role involves regular physical activity, including lifting patients over 200 lbs with assistance, transporting supplies and equipment up to 20 lbs for distances of 500 feet, and frequent standing, walking, sitting, stooping, and reaching. Responsibilities: Conducts comprehensive nursing assessments and collaborates with patients, families, physicians, and the Interdisciplinary Group (IDG) to develop, implement, and evaluate individualized plans of care in alignment with regulatory requirements and the Case Management Recipe. Maintains timely and accurate documentation, ensures care plans reflect current patient status, and communicates effectively with the care team to coordinate optimal use of resources, especially in complex or high-risk situations. Provides education and emotional support to patients and caregivers based on individual needs, and actively participates in on-call rotations, admissions, and care transitions as directed. If serving as a Care Manager, coordinates all aspects of patient care including scheduling, medication management, plan of care oversight, IDG collaboration, and supervision of LPNs, LVNs, and Hospice Aides; participates in IDG meetings and patient/family conferences. If serving in an On-Call capacity, responds to triage calls, performs visits and assessments, coordinates care with the Administrator On Call, and may attend IDG meetings and family conferences; also participates in training, committees, and other duties as assigned within licensure scope. Objective The MDS Coordinator assists the Director of Nursing and the RN Assessment Coordinator with ensuring that documentation in the center meets Federal, State, and Certification guidelines. The MDS Coordinator coordinates the RAI process assuring the timeliness, and completeness of the MDS, CAAs, and Interdisciplinary Care Plan. Principal Responsibilities Assists the center in assuring adherence to Federal and State regulations and certification. Actively participates in the regulatory or certification survey process and the correction of deficiencies Reports trends from completed audits to the Quality Assurance Committee Assures the completion of the RAI Process from the MDS through the interdisciplinary completion of the plan of care. Initiates and monitors RAI process tracking, discharge/reentry and Medicaid tracking forms through the PointClickCare system. Follows up with staff when necessary to assure compliance to standards of documentation. Completes patient assessments, data collection, and interviews staff as necessary to assure good standard of practice and as instructed in the current version of MDS User’s Manual. Facilitates accurate determination of the Assessment Reference Date that accurately reflects the patient’s care needs and captures all resources utilized to ensure appropriate payment by Medicare/Medicaid and insurance programs. Ensures timely submission of the MDSs to the State with proper follow-up on validation errors. Maintains validation records from the submission process in a systematic and orderly fashion. Qualifications Graduate of an approved Registered Nurse / License Vocational Nurse program and licensed in the state of practice required. Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. Excellent knowledge of Case- Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required. Through understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set. Knowledge of the care planning process.
