Educate patients and caregivers to appropriately infuse home intravenous or subcutaneous medications in the home setting. Responsible for safe administration and monitoring of patient for intravenous medications required to be administered by nurse per departmental policy (i.e. biologics). Home Environment and Care Coordination. Assesses home environments for safe medication administration. Ensures accurate documentation and appropriate plans per accreditation standards. Obtains venipuncture laboratory specimens in accordance with best practice. Coordination, and collaboration of care for patients to ensure care is provided in a safe, effective fashion. Incorporates evidence-based nursing practice taking into consideration current statutes, rules and regulations when developing the plan of care. Patient Outcomes and Quality Improvement. Collaborates with patients and families to set realistic, measurable outcomes. Participates in quality improvement strategies and to improve care quality and efficiency. Implements interventions in a safe, timely, appropriate manner and escalates the patient to higher level of care if indicated. Professional Practice Evaluation. Evaluates one’s own nursing practice in relation to professional practice standards and regulatory guidelines.
