Job Description The Clinical Registered Nurse (RN) II is responsible for managing individualized patient care by promoting and restoring patients' health in accordance with medical and nursing plans of care and established policies and procedures. The Clinical RN II collaborates with physicians and multidisciplinary team members in the development, execution, and evaluation of the multidisciplinary plan of care. The RN assumes a full patient assignment and can plan, analyze, intervene and set goals for patient care accordingly. This nurse provides nursing services to patients and families in accordance with the scope of the RN as defined by the Virginia Board of Nursing. This RN maintains a safe care environment for their patients (clients) and delivers quality care. Responsibilities Clinical - Assessment Conducts an individualized patient assessment, prioritizing the data collection based on the patient’s immediate condition or needs within timeframe specified by policies, procedures or protocols. Conducts ongoing assessments as determined by patient’s condition and/or the policies, procedures or protocols and reprioritizes care accordingly. Integrates nursing interventions into the multidisciplinary plan of care, incorporating appropriate standards of care and practice and patient outcomes. Clinical - Planning Develops, modifies, and integrates an individualized care plan which incorporates the medical plan of care, key elements from the plans of other disciplines, and standards of care and practice. Establishes individualized, measurable patient goals in consultation with the patient, family, and health care team. Clinical - Intervention Performs appropriate treatments as ordered by provider in an accurate and timely manner. Performs therapeutic nursing interventions as established by individualized plan of care for the patient and his/her family. Recognizes potential problems and responds to general information/data indicating urgent and/or emergent risk to the patient. Initiates emergency resuscitative measures as appropriate. Assumes responsibility and accountability for effectively managing the care of a specialty patient population. Provides care in a non-judgmental, non-discriminatory manner that is culturally sensitive to the patient and family, preserving their autonomy, dignity and rights. Collaborates with interdisciplinary team members to ensure patient care interventions are appropriate. Clinical - Evaluation Evaluates patient’s responses to care based on effectiveness of nursing interventions/actions in relationship to established goals. Initiates changes in the interdisciplinary care plan based on this evaluation. Problem solves with the specialty team to improve patient outcomes. Documents patient assessment findings, physical/psychosocial responses to nursing intervention and progress towards problem resolution. Clinical - Patient Education Identifies learning needs of patients/families and teaches by adapting standard information based on needs. Ensures that the patient/family demonstrates knowledge, within the abilities of the patient. Communicates with patients and family to identify their needs and include them in the plan of care. Acknowledges and supports determinations by the patient and/or family concerning treatment. Self and Peer Learning Assumes responsibility for assessing and meeting own learning and professional development needs. Maintains current competency in nursing specialty and requirements for department; attends required educational programs and may attend additional educational workshops; reviews nursing professional publications; establishes personal networks; may participate in professional societies. Seeks additional educational opportunities for clinical development and is either ready to learn the charge nurse role or currently functioning in a charge nurse role. Collaboration Makes sound judgments in the decisions involved in coordinating multiple, increasingly complex patient care demands. Supervises delegated care based on abilities of personnel and patient acuity. Reports changes in patient condition to appropriate personnel. Service Recovery Participates in service recovery and uses appropriate resources during complex situations. Handles conflict resolution in an appropriate, timely and consistent manner. Assists in the orientation, instruction, direction, and evaluation of students assigned to the unit. Able to be trained for a charge nurse role or is currently functioning in a charge nurse role. Essential Physical Requirements: Ability to transfer and/or maneuver objects weighing at least 50 pounds in the assessment and implementation of patient care. Requires frequent pushing, moving, lifting of patients. Positioning of patients, giving patients baths and ambulating patients expending much physical effort. Occasionally requires reaching overhead, stair climbing and fine motor manipulation. 7. Essential Mental Abilities: Must be able to assess a patient’s condition, formulate a plan of care, select appropriate interventions, evaluate patient’s response to care/treatment, and to explain/teach patients about their condition/recovery. Requires higher level of mental faculties accompanied by short-and long-term memory. Able to prioritize duties, learn new skills and techniques in patient care. Able to learn and use supportive services. 8. Essential Sensory Requirements: Ability to visually assess patients and to utilize sight to implement and evaluate plan of care (changing dressings, starting IVs, regulating IV’s, maintain equipment as to readouts, etc.). Utilize hearing to auscultate lung sounds, bowel sounds, hear alarms, and effectively communicate with patients, families, physician, and staff. 9. Exposure to Hazards: Noise, exposure to blood borne pathogens and body fluids, infectious diseases, and needle puncture wounds. May be exposed to dangerous animals and traffic hazards while home visiting. May encounter patients and other situations which present a potential threat to personal safety. Plan, conduct and participate in In-service Education Programs. Assist in implementing and maintaining an effective orientation program that orients the new Ladies and Gentlemen of Alden to the facility, his/her department and its policies and procedures. Attend and participate in continuing education programs. Place orders for equipment and supplies as necessary following established budgetary guidelines. Maintain budgetary restraints by following established procedures for preparing purchase orders for review by the Administrator prior to placing orders. Maintain a reference library of written nursing materials (i.e., Drug Formulary, State and Federal regulations, etc.) that will assist the nursing service department in meeting the day-to-day needs of the customer. Participate with the Administrator and Admissions Director in selecting appropriate admissions and assignments of rooms. Review nurses’ notes/EHR to ensure they are informative and descriptive of the nursing care being provided, and they reflect the customer’s response to the care. Responsibilities for Registered Nurse (RN): Provide resident care in accordance with acceptable standards of nursing practice and the physician’s plan of treatment, as well as the written plan of care. Follow and implement care plans developed by the Nurse Manager. Perform medication administration and treatments in accordance with the plan of care (POC). Monitor and assess residents’ conditions, promptly reporting changes to appropriate clinical team members. Maintain accurate, timely, and complete documentation in accordance with physician orders, facility policies, and regulatory standards. Collaborate with interdisciplinary teams to ensure coordinated and high-quality resident care. Ensure compliance with infection control protocols, safety standards, and nursing regulations. Qualifications for Registered Nurse (RN): A current, unencumbered active license to practice as an RN in the state of MA. Valid BLS Certification (CPR) required. Ability to manage care for multiple residents simultaneously in a fast-paced healthcare environment. Strong clinical assessment, communication, and documentation skills. RN (Registered Nurse) Job Duties: Assists in defining, maintaining and interpreting standards of practice. Performs required skills (assessment, intervention) according to guidelines, policies and procedures. Participates in interdisciplinary committees which influence or determine policies affecting nursing practice. Follow facility procedures/protocols for admitting residents transferring to hospitals/clinics/outpatient visits and completion of appropriate documentation based on established guideline. Makes rounds on all resident rooms and ensure that the L.P.N.’s, C.N.A.’s and/or others under his/her supervision receive their resident reports, has their assignments and break times scheduled. Assist the L.P.N. in establishing routines which facilitate a high level of nursing practice. Receives and give reports from and to outgoing and incoming shifts. Count or ensure the counting of controlled drugs with outgoing and incoming shifts. Coordinates/Prepares MDS schedules for quarterly, annual and significant change assessments. Complete MDS Booklet as per schedule (Nursing Sections). Reviews and signs off on all completed MDS and RAP Summary. Develops care plan that demonstrate knowledge of criteria to ascertain resident specific needs based on health care problem(s). Makes needed revisions and updates to the care plans. Ensures that care plan is carried out which demonstrates familiarity with criteria to establish priorities specific to anticipated outcomes. Monitors the provision of care for the residents in his/her assigned unit. Interprets philosophy and objectives of nursing service to residents, families and others. Identifies resident/family learning needs based on the discharge care plan as applicable and carries out appropriate teaching required. Demonstrates ability to coordinate and collaborate with resident/family and other members of the comprehensive health care team. Monitors the administration and accurate recording of medications used. Investigates and follow through on medication errors. Notifies physicians of pertinent observations and reactions, incident and abnormal laboratory and x-ray results as required and documents same promptly. Transcribes physician orders timely and ensure orders are implemented as appropriate. Reports all resident, visitor and work-related accidents/injuries and demonstrates appropriate follow through by investigating, documenting and processing incident reports. Accepts guidance, supervision, and criticism if needed. Practices in compliance with the Nurse Practice Act and regulatory agencies regulations. Assures that a stock level of medications, medical supplies, equipment, etc. are maintained at adequate levels at all times. Assume the authority, responsibility and accountability of directing your nursing shifts. Other responsibilities as deemed necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator.
