Organization: VCH Step Down Job Summary: Responsible for planning, organizing, and administering patient care within the scope of his/her training and expertise, collaborating with other professional disciplines to ensure effective and efficient patient care delivery and achievement of desired patient outcomes is met. This is done in conjunction with the objectives and policies of the hospital and the Nurse Practice Act of the State of Tennessee. . Hours of Position: Full-time (36 hours/week), days (6:45a-6:45p) Stepdown is a 21 bed hardwired telemetry unit that focuses on acutely ill but stable patients requiring closer monitoring. We care for patients with a variety of cardiac, vascular and medical diagnoses with specialized needs such as cardiac arrhythmias, bedside cardioversions, titrated medications, oxygenation management, withdrawal, sepsis, stroke, and post-operative care. Our staff is dedicated to provide excellent care to our patients. Our environment is challenging, fast-paced, team oriented, and patient and family centered. Our staff is very engaged with problem-solving and identifying ways to improve patient care. They have a desire to continue learning, have strong communication skills, and collaborate with the team to provide excellent patient care. Daily interdisciplinary rounds include the provider, registered nurse, case manager, and a nurse leader. Each shift begins with a safety huddle to look back and forward 24 hours to review safety and quality concerns. Ratio 4:1 Certifications preferred: BLS, ACLS, NIH Orientation: We offer 8-12 week orientation based on nursing experience. Professionalism: critical thinking, collaboration, multidisciplinary rounding, bedside shift report, shared governance, certifications. Medical equipment: ECG hardwired, alaris, pca, epidural, Etc02, bladder scanner, bipap, high flow oxygen therapy. Drips: cardiac, IV fluids, blood products, sedation, TPN KEY RESPONSIBILITIES Collaborates in establishing the plan of care, coordinates and implements care delivery while anticipating patient care needs across the continuum. Analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis. Continually utilizes the nursing process to evaluate patient's progress towards goals and applies appropriate interventions to ensure effective outcomes. The RN Home Care works autonomously to provide safe, thorough and effective physician-ordered care in the home setting. PURPOSE AND SCOPE: Functions as part of the hemodialysis health care team as a Staff Registered Nurse to ensure provision of quality patient care on a daily basis in accordance with FMS policies, procedures, and training.
