Prepares the patient for operative procedure by positioning and grounding the patient as required, securing the patient on the table, assisting with intubation, and preparing operative site. Accompanies the patient to recovery room and may be required to provide nursing care in the recovery room. Monitors operative procedures by documenting proper patient positioning, breaks in sterile procedure, instrument, needle and sponge counts, preservation and labeling of all specimens, drugs and blood administration, and critical incidents during the operative procedure. Functions as a team member within the surgical department by assisting with gowning and gloving; procuring required medication, blood and supplies; coordinating involvement of other departments with the procedure; properly processing operative specimens. Maintains a safe and productive work environment by assisting with cleaning, maintenance, and re-supply. Successful completion of a training program and demonstrated competence in circulating nurse duties as determined by the Director of Surgical Services. The nurse shall be adequately oriented to unit policies, procedures and equipment. Orientation shall include safety procedures, infection control practices, and performance improvement activities. Job Summary GENERAL SUMMARY We are seeking a highly skilled, compassionate Registered Nurse (RN) to deliver high-quality, patient-centered care through a combination of in-home patient visits, telehealth patient consults with Nurse Practitioners or Physicians as required, and administrative tasks. Ensure compliance with all regulatory standards, facility policies, and procedures related to respiratory care. Facilitates the development and implementation of a comprehensive, multidisciplinary plan of care for patients with complex medical and socio-economic problems; provides real time interventions to ensure compliance with health care regulations; meets with new admission patients to identify continuum and discharge needs; ensures documentation of initial discharge planning assessment within the first 72 hours. Identifies and documents proper level of care; discusses cases with attending and physician advisor; identifies anticipated length of stay; works with attending on clinical pathway or MCG guideline; proactively assists in resolving barriers; develops and implements case management plans and interventions; performs discharge planning; completes responsibilities by following established guidelines and protocols within the appropriate timeframe. Initiates the discharge plan and documents interventions; updates care plan weekly; confirms Face Sheet information; assures proper hand off for transfers; communicates discharge plan to new case manager; assesses need for referrals to outside resources; obtains authorizations for Home Care, Home IV infusion and DMEs; coordinates forms or electronic referrals; arranges transportation; meets departmental performance expectations for home care/hospice and equipment; confirms services and contact information. Identifies readmissions within 30 days; performs walking rounds and identifies high‑risk patients; works with Social Work and utilization management to reduce LOS and prevent denials; coordinates and manages distribution of IMM and assists with MOON; conducts or co‑leads care coordination rounds; promotes working relationship with patient/family; acts as advocate; works closely with Social Work and utilization management; serves as a resource regarding patient classification and reimbursement. Overview About the Role The Clinic Nurse Manager is an experienced clinical leader responsible for managing assigned areas and multiple services within the assigned practice, supervising assigned patient care personnel during clinic operations. The Clinic Nurse Manager is responsible to ensure the safe and effective delivery of patient care and clinical outcomes, compliance with regulatory, administrative, and clinical standards, policies, and procedures. The Clinic Nurse Manager is seen as an expert in clinical practice topics and is proficient in personnel management and directs care to meet the unit demands in an efficient and cost-effective manner, as well as assisting with clinic budget management. This position reports to the Administrative Director of the practice and the Executive Director Clinical Operations. Responsibilities What You’ll Do Maintains nursing and clinical operations by initiating, coordinating, and enforcing program, operational and personnel policies and procedures within the assigned practice in the Ambulatory Care Center. “Owns” clinic operations and exercises broad oversight for clinic patient throughput, provider performance, and clinical and/or support services. Maintains accountability for daily staffing patterns and appropriate skill mix to support the practice’s volume and acuity. Assures quality of care by ensuring adherence to state board of nursing practice act requirements and other governing agency regulations including National Patient Safety Goals, measuring health outcomes against standards, and making or recommending adjustments in practice. Qualifications What You’ll Bring Required: Licensed as a Registered Nurse in the State of New Jersey. Bachelor’s Degree in Nursing (BSN) required, a Master’s Degree (MSN) preferred. A minimum of five (5) years of nursing experience required. BLS (CPR) is required. Certification as an Ambulatory Nurse (RN-BC) is desirable and preferred within two (2) years of hire. Must have the ability to lead a team and maintain maximum effectiveness and efficiency. Strong oral and written communication skills, and exceptional customer service skills required.
