The Registered Nurse utilizes the nursing care process consistent with their license and/or certification including initiation of culturally competent continuum of care including inpatient, outpatient and community settings. The Registered Nurse is responsible for overseeing any necessary coordination of the interdisciplinary plan of care and provides for its consistency with the medical treatment plan. The patient care process includes: * Assessment - an orderly process consisting of the identification, gathering and organization of subjective and objective data pertaining to the patient/family/significant other, including physical, psychosocial, spiritual and cultural. * Planning - an organized step in the process that consists of the identification of actual or potential problems and desired outcomes (goals) that result in a documented plan of care. The plan of care provides direction for the patient/family/significant other and interdisciplinary team. * Intervention - the specific interventions or orders that are implemented consistent with the plan of care and medical treatment plan. * Evaluation - a step in the process that involves determining the effectiveness of the interventions in achieving the plan of care and determining the conclusions concerning the achievement or lack of achievement of desired outcomes. The evaluation may initiate the need for reassessment and revisions to the problems, goals, plan of care, and interventions. This step provides for documentation of required actions. Based upon findings identified during assessment and reassessment: * Patients have problems identified and prioritized relative to biophysical, psychological, environmental, self-care, educational and social status. * Goals are identified consistent with the problem to provide direction for patient care and health promotion. * Education is consistent with the identified problem and associated goals and is seen as critical to creating an environment that promotes patient empowerment * Employee may be required to perform Limited Laboratory Services to include patient identification and preparation for specimen collection, processing samples for testing, performing required quality control testing and instrument maintenance and function checks, monitoring, recording and reporting tests results to clinicians to include critical values, and maintaining competency/certification of the aforementioned tasks and duties. Licensed and registered care providers responsible for utilizing the care process relative to their scope of practice and knowledge base, may document the assessment, plan, intervention and Evaluation. QUALIFICATIONS EDUCATION/LICENSE: * Graduate of an accredited school of nursing; BSN preferred. * Current Licensure as a Registered Nurse in the State of New Jersey or current multi state license required. * American Heart Association BLS required upon hire or transfer. Description Cornerstone Specialty Hospitals Round Rock is a long-term acute care (LTAC) hospital specializing in the treatment of patients recovering from post-intensive care and medically complex conditions. Our hospital provides 3 different levels of care to include LTACH, inpatient rehab, and outpatient wound care. Our hospital provides both intensive care and telemetry-level services, offering advanced monitoring and support for patients requiring extended medical care. With a multidisciplinary team dedicated to personalized treatment plans, we focus on helping patients achieve significant recovery and transition to the next level of care. Our 12-bed acute rehabilitation unit is designed for patients who are ready to progress to intensive therapy for 15 hours a week. **Hospice Experience Required** **RN License Required** JOB DESCRIPTION SUMMARY As an Admissions Nurse, you are responsible as a member of the Interdisciplinary Team for the provision of skilled nursing assessment, planning and care on an on-call basis in order to maximize the comfort and health of patients and families consistent with Hospice policies & procedures. You will collaborate with the patient, caregivers, clinical manager, case managers, and physician(s) to complete the patient care planning at the onset of care. You will provide skilled interventions and instructions to the patient and caregivers to provide optimum symptom control and patient comfort. You will function as a liaison between disciplines during the admission process. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of the nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. As a mandatory reporter, reports failure to comply with the requirements of the Ohio Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice in the state of Ohio 3.
