Facilitates coordination between multidisciplinary team and patient/family/caregiver for care conferences when indicated. Develops treatment plan and discusses proposed course of treatment with patient’s attending physician, patient, family/caregiver, and other members of the multidisciplinary team. Provides discharge education to patient families. Completes enrollment of qualified infants into the High-Risk Perinatal Program (HRPP) according to the Arizona Department Health Services criteria. Coordinates inpatient Retinopathy of Prematurity (ROP) Screening Program for at-risk NICU patients. Arranges discharge medical equipment, supplies, and medications and arranges or provides related training as indicated. Maintains Interdisciplinary Rounding List for discharge planning. Conducts discharge follow up calls for identified patients/families. Minimum Qualifications EDUCATION: Bachelor's degree in nursing or associate degree in nursing. LICENSURE OR CERTIFICATION: Current RN licensure permitting work in state of Arizona and basic life support (BLS) required.
