Prepares, administers and documents medications and treatments as prescribed in the Physician’s Plan of Care. Verifies current medications with medication profile. Evaluates the patient/family response to the Plan of Care against established goals. Provides, establishes, and revises the Home Health Assistant Plan of Care to Home Health Assistants as assigned. Current license as a Registered Nurse in the State of Michigan. Two years of recent clinical experience, preferably in hospice, home care, oncology, gerontology, or surgical nursing required.
