Recognizes signs and symptoms of potential emergencies to include behavioral and physical changes. Initiates appropriate actions and timely interventions. Recognizes indicators and proper reporting of abuse. Practices National Patient Safety Goals (NPSG), see JCAHO Reference. Utilizes appropriate methods to access risk and prevention patient falls & skin break down on intact skin. Assessment/alternative for management of restraints. Performs and documents legibly/timely and accurate nursing interventions to meet patient care needs. Coordinates total nursing care to the patient using the plan of care to provide continuity of care. Assumes the responsibility for teaching and documenting patient education appropriate and consistently. Assures complete follow-up instructions are given with demonstration or verbalized understanding by the patient prior to discharge and documented in the medical record.
