Ensures that nursing diagnoses are congruent and collaborative with medical diagnoses. 3. Identifies expected outcomes individualized to the patient. Makes independent nursing judgments recognizing need to change plan of care based on patient outcome. 4. Prioritizes patient’s actual and potential needs. Forms plan of care in collaboration with the patient, significant others, and health care team members. Adjusts the plan of care according to the continual assessment of the patient. 5. Anticipates impending crisis and initiates early intervention to prevent problems or complications. Performs advanced skills specific to care of the critically ill/injured patient, both in a hospital setting and in the transport environment. 6. Evaluates and modifies the nursing plan of care based on observable responses to interventions and attainment of patient outcomes. Ensures the evaluation is systematic and on-going.
