Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care. Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.). Provides appropriate support at time of death and perform bereavement assessment. What is Required? Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing Current State License as a Registered Nurse RN 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Valid driver’s license and insurance and reliable transportation to perform job tasks You will report to the Clinical Team Manager. INCU has critically care-trained RNs who support the patient's needs and care for ventilated patients. South 10 also has 2 negative pressure isolation rooms with an anti-room that is used when needed for specific isolation needs. We do multidisciplinary rounds Monday – Friday. This involves the patient in their care and allows the team members to collaborate with the patient to give them the best possible care and appropriate discharge to home or other facilities. We have a pharmacist who has an office on the unit to assist the RNs when needed to help avoid delay of care to any patient. Education Requirements Graduate of NLN accredited School of Nursing.
