We are seeking a Registered Nurse (RN) to: Provide hands-on acute care in the home setting, delivering hospital-level interventions including IV medication administration, lab draws, wound care, advanced assessments, and patient education. Care for patients enrolled in the Home-Based Hospital Care program, utilizing technology such as remote patient monitoring, video visits, and mobile diagnostic services to support safe, high-quality care at home. Collaborate closely with an interdisciplinary care team, including providers, program coordinators, paramedics, and support staff to ensure seamless, patient-centered care. Perform comprehensive patient assessments, evaluating physical, functional, psychosocial, and cognitive status while monitoring response to treatment in the home environment. Responsible for nursing care planning and management of patients in collaboration with the multidisciplinary team. This is a non-supervisory position following the Florida Nurse Practice Act and chain of command. This position comes with a 1 year commitment, with a 1 year option for renewal. The rate of pay for this role is $55/HR. Job Responsibilities: Utilizes basic physical and psychosocial skills by identifying actual/potential nursing/clinical problems based on bio-psycho-social- spiritual assessment data of the client and their families or caregivers and documents findings.
