Assesses patient needs and makes care recommendations based on the patient’s physical, psychosocial and cultural needs, age and functional status. Facilitates timely patient throughput optimization for all phases of care including admissions, transfers, and discharge optimization of patients on the designated unit. May participate in plan of care rounds depending on service and unit protocols. Provides timely Smart Square schedules and on call schedules if applicable. Follows up appropriately and effectively resolves customer and physician issues and complaints. Monitors staff schedules to ensure adequate staffing to meet patient care needs and maintain quality patient care. May authorize overtime or scheduling changes to fill shifts as necessary. Completes payroll in collaboration with Nurse Manager. Provides volumes & adjustments to reflect variances in staffing needs within Smart Square. Collaborates with the Nurse Manager to evaluate, maintain and improve patient care team member performance. Assists in implementing changes in clinical practice and evaluates the quality and effectiveness of nursing practice to ensure compliance with organization, department and unit specific goals. Supports nursing professional development, Magnet, and Shared Governance through participation in, and mentoring of colleagues, on councils and the Clinical Ladder program. Provides patient care support when necessary. Minimum Qualifications Specialized Diploma Graduate of an accredited school of nursing required. 3 years as a Registered Nurse in an acute care setting. Effective Communication- Understanding of effective communication concepts, tools and techniques, ability to effectively transmit, receive, and accurately interpret ideas, information, and needs through the application of appropriate communication behaviors. Uses the language of partnership with patients and families, engaging them in their health and health care. Medication Administration and Prescription Knowledge of various medications used to treat patients, ability to apply this knowledge to the administration of medications and to the prescription of drugs for continuing treatment outside of the hospital. Discharge Planning Knowledge of the patient's post-discharge needs and the ability to consider these needs in planning for discharge and arranging for follow-up care. Assessment and Diagnosis Knowledge of various tools and techniques for patient assessment and nursing diagnosis or clinical judgement, ability to explore and analyze reasons behind symptoms and generate resolutions according to available drugs, facilities and techniques. The Pediatric Allergy/Immunology Pediatric Nurse Practitioner or Physician Assistant delivers comprehensive independent and collaborative care as an integral member of the Allergy/Immunology team in Wilmington Delaware. The Pediatric Nurse Practitioner/Physician Assistant will play a key role in the development of an Oral Immunotherapy (OIT) program in Wilmington, DE. A majority of clinical time will be dedicated towards oral immunotherapy treatment. Additionally, they will be delivering outpatient care for pediatric patients requiring diagnosis, evaluation, and management of other Allergy/Immunology concerns. Essential Functions Practices collaboratively with physicians and other health care providers to provide high quality health care to children. Performs appropriate assessment of patient/family upon referral/admission. Performs ongoing assessment and evaluation of patient progress and revision of interdisciplinary plan of care. May use prescriptive authority if appropriately credentialed to do so. Collaborates inter-professionally as needed. SUMMARY OF JOB This is a full-time opportunity to work at Redeemer Valley Farm, a ministry of the Sisters of the Redeemer in Huntingdon Valley, PA A Livestock caregiver on this regenerative farm would oversee daily livestock care in collaboration with the Farm/Dairy Coordinator, including flock of chickens, facility upkeep, and manure management to actively restore soil health, enhance local biodiversity, and mimic natural ecological cycles. Core responsibilities: *Herd Health and Husbandry: Manage daily care, nutrition, hoof trimming, vaccinations, and structural healthcare for herd of dairy goats following animal welfare and safety guidelines. Care of chickens and coop house for healthy production of eggs which are sold at the weekly market. *Milking & Creamery Prep: Milk goats, clean and sanitize daily milking operations, maintaining sanitation standards for raw and pasteurized milk production and assist in cheese production when necessary. *Regenerative Pasture Management: Create intensive rotational grazing schedules to simulate, forage growth, build, topsoil, and prevent pasture degradation. *Kidding and Breeding Management: Co-facilitate with dairy leader in breeding season and provide hands-on care during kidding, including neonatal care and kid rearing. *Record Keeping: Track comprehensive data on Milk yield, breeding, lineages, health, interventions, and rotational grazing timelines. *Facility Upkeep: Maintain barn cleanliness, predator proof fencing, milking equipment, and composting systems for deep litter bedding. Order and maintain inventory of animal feed, bedding and supplies for all animals. Works cooperatively with Farm/Dairy Coordinator and Volunteer Coordinator.
