Job Description The Nurse Practitioner 2 will work as part of a Team at the Mission Bay and Mt Zion locations within the UCSF Department of Radiation Oncology in a multidisciplinary environment working with Radiation Oncologists, Residents, Registered Nurses, Medical Physicists, Radiation Therapists, Dieticians, and Social Workers in collaboration with referring medical oncologists and surgeons. The NPII reports to and is managed by an APP Manager while working under the supervision of Faculty/Attending Providers. The patient populations may vary but will focus primarily on Solid Tumor cancers. The incumbent will provide care to patients including taking histories; conducting physical exams; document findings and actions as indicated, complex wound care within the department and assist with planning of the course of patients’ treatments for adult populations. The NPII will perform site-specific patient teaching; prepare patients for simulation; may assist with contouring structures for treatment planning; medicate patients as needed; order medications for symptom relief; assist in managing patients in follow-up; and address quality of life issues not limited to skin/wound care, speech/swallowing, adequate nutritional intake; and psycho-social issues. In addition, the NPII will answer patient phone calls, emails, My Chart messages, questions and perform other duties as required such as attending tumor boards, participate in department, and medical center committees as needed. Other duties may include developing patient education materials; staff development; training; teaching; outreach; assisting with research; analyzing or collecting data; and assisting other service teams in providing care to patients in the department as needed. The Nurse Practitioner provides primary care to enrolled participants by evaluating participant physical complaints and providing appropriate treatment. The Nurse Practitioner functions beyond as an advanced practice nurse and as an integral member of the interdisciplinary team. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties. Evaluates participants and documents information in participant’s chart. Performs physical examination and orders and interprets diagnostic tests. Performs blood draws, EKG’s, dressing changes and any other nursing interventions as needed. Formulates an assessment of participant’s condition and provides a treatment plan, including writing prescription(s) and providing ongoing follow-up of condition. Provides alternative methods of therapy to participants as appropriate. Educates staff, participants and caregivers regarding healthcare issues and treatments. Manages acute and long-term medical needs of participants in collaboration with other primary care providers and oversees the care eds of participants in nursing homes, homes, Assisted Living Facilities and/or group clinic settings. Provides emotional and spiritual support to participants. Provides management of medical emergencies as needs dictate. Ensures appropriate and timely documentation of medical care in the participant’s medical record, in compliance with InnovAge Greater PACE documentation policies. Identifies documents and corrects problems in medical records.
