In this role, the Registered Nurse will: Provide routine daily direction and monitor the work of the LPNs and nursing assistants on the unit, assuring a high standard of nursing care and satisfactory completion of tasks. Assist in the orientation of new nursing personnel and reinforce training standards to maintain a high quality of care. Assist in motivating LPNs and CNAs assigned to the nursing unit. Provide explanation of nursing procedures and assistance to staff to assure ongoing understanding and training. Communicate on a regular basis with the Director of Nursing regarding assessments, observations, and suggestions of problem areas, resident and employee activities, concerns, needs, conditions, etc. Observe and evaluate residents' health, mental and emotional conditions and communicate significant changes to appropriate staff. When indicated is responsible for notifying the physician and family members according to prescribed procedures. The ideal candidate will have: Graduate of an approved school of nursing. Current license as a Registered Nurse in the State. One year nursing experience necessary. Devotion to standards and principles of professional nursing. Skill in assessing problems, planning and evaluating solutions in relationship to routine supervision of LPNs and nursing assistant and resident needs. Understanding, patience and tact in dealing with people. Must be tactful and possess excellent communication skills (written and verbal, English). Genuine compassion for senior adults and the disabled. Requires good human relation skills and the ability to monitor and direct the activities of others. Principal Responsibilities • Works in collaboration with the Interdisciplinary Team to assess the needs of the resident; Provides interdisciplinary schedule for MDS assessments and care plan reviews as required by governing agencies. • Ensures that the Interdisciplinary team makes decisions for either completing or not completing additional MDS, assessments based on clinical criteria as identified in the most recent version of the RAI User’s Manual. • Assist with coordination and management of the daily stand up meeting, to include review of resident care and the setting of the assessment reference date(s). • Complies with federal and state regulations regarding completion and coordination of the RAI process. • Monitors MDS and care plan documentation for all residents; ensures documentation is present in the medical record to support MDS coding. • Maintains current MDS status of assigned residents according to state and federal guidelines. • Maintains the frequent and accurate data entry of resident information into appropriate computerized MDS programs. • Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident and family members. • Attends interdisciplinary team meeting, quality assurance and other meeting in order to gather information, communicate changes, and maintain and update records. • Assists DON or designee with identification of a significant change, physician orders and verbal reports to assure that the MDS and care plan are reflective of those changes. • Prepares scheduling, notice of resident care planning conferences, and assists DON in communication of outcomes/problems to the responsible staff, resident, and/or responsible party. • Continually updating knowledge base related to data entry and computer technology. • Completes electronic submission of required documentation to the state database and other entities per company policy. • Corrects and ensures completion of final MDS and submits resident assessment data to the appropriate State and Federal government agencies. • Assigns, assists, and instructs staff in the RAI Process, PPS Medicare, Medicaid (Case Mix as required) and clinical computer system in relation to these processes. • Maintains confidentiality of necessary information. • Other duties, responsibilities and activities may change or assigned at any time with or without notice. Acute Care Medicine has patients with a full range of clinical issues-from significant blood dyscrasias, advanced endocrine pathology, heart and renal failure, GI conditions, infectious diseases, asthma and a multitude of other conditions and diseases.¿ Due to the patient acuity on the nursing division, we provide advanced training to bring new nurses to the highest level of functioning.¿ Each unit is lead by a clinical manager and lead charge nurse.¿ The unit is supported by an advanced practice nurse.¿ Additionally each unit is assigned a case manager, social worker, physical therapist, dietitian, and pharmacist.¿ This multidisciplinary team works in conjunction with the physicians, nurses and patient care technicians to provide quality patient care. Critically assess patient data to proactively mitigate impending changes in condition and establishes consults to interprofessional team as appropriate. Leads the development and implementation of complex individual plans of care with defined goals in collaboration with other members of the interprofessional team and the patient, family, or caregiver in accordance with the established guidelines and standards of nursing care. Plan of care reflects application of expert holistic clinical knowledge and skill to anticipate and prioritize complex care needs, anticipate barriers, incorporating interprofessional team and caregiver/family input, and identifies opportunities to improve patient outcomes. Actively utilizes and champions scientific inquiry for evidence-based practices (e.g., participates in journal club, facilitates practice change based on new knowledge, recommends new standards of care or revisions to current standards, conduct a literature search to support unit or organizational goals, disseminate new knowledge obtained from relevant conference or continuing education). Explore opportunities to advance nursing practice (e.g., support a community service health event as a representative of the healthcare profession, patient/family education group, community advisory board, member of Ethics Advisory Committee, serve as adjunct faculty, leadership board in specialty organization). Analyzes gap in practice standards and provides feedback to peers in collaboration with leadership to enhance safe patient care.
