Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members. What is Required? Current, unrestricted state RN license. 1+ year experience in a clinical care setting as a nurse. Associates Degree or Bachelor’s Degree in Nursing is preferred. Previous Home Health or Hospice Experience is preferred. Previous experience with HomeCare HomeBase is preferred. Able and willing to travel within the branch/office coverage area. Current licensure in State of CA and CPR certification. 3. Registered nurses / Licensed Vocational nurse shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN / LVN license. Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 6. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. JOB PURPOSE : The Registered Nurse serves as part of the team providing quality healthcare to El Rio Health patients. In this role, the Registered Nurse provides direct patient care, both clinical and non-clinical within the scope of practice for a Registered Nurse under the Arizona State Board of Nursing and policies and procedures set by El Rio Health. The Registered Nurse utilizes the nursing process, collaborating with the physicians and interdisciplinary team members and provides physical and psychological support to patients and their families.
