Summary The Performance Improvement Coordinator is an integral member of the Trauma Registry at Rhode Island Hospital. As the only ACS verified Level I Trauma Center in the state of Rhode Island, we provide essential trauma care to nearly 4,000 patients with traumatic injuries every year. The Trauma service relies on the Performance Improvement Coordinator to coordinate the performance improvement process that is integral to our hospital’s American College of Surgeons verification status. Under the direction of the Trauma Program Manager, the Performance Improvement Coordinator identifies performance issues, assures timely evaluation, assists with developing action plans, assists with implementing those plans and is responsible for evaluating effectiveness. Identify opportunities to improve care of the trauma patient through attendance at divisional meetings and document these issues in TraumaOne, the Trauma Registry database. Evaluate and validate Performance Improvement issues/concerns. Collaborate with various disciplines and departments to develop action plans of identified issues. Present the issues, evaluation and action plan to TPM, TMD and Trauma Committee members. Assists in implementing the action plan and monitors its effectiveness through the PI Process and report’s findings to the TPM, TMD and Trauma Committee. Ensures completion of action plan items and creates remediation plans if necessary to ensure loop closure on identified opportunities of care. Maintains the documentation of these PI issues and plans in an organized, traceable format. Establishes Performance Improvement filters to monitor processes within the Division of Trauma. Develops focused review presentations for PI filters that exceed established thresholds and presents to Trauma Committee. Actively participates in ongoing preparation for the American College of Surgeons (ACS) site visit. Coordinates the Continued Medical Education events with the TPM and TMD. Collaborates in a professional manner with all members of the healthcare team to promote the goals and objectives of the Division of Trauma. Participates in professional development/educational activities to ensure advancement of skills and knowledge of trauma care and data information systems. Complies with all institutional and departmental policies, procedures and objectives. PURPOSE AND SCOPE: Functions as part of the hemodialysis health care team as a Staff Registered Nurse to ensure provision of quality patient care on a daily basis in accordance with FMS policies, procedures, and training. Provides safe and effective delivery of care to patients with ESRD. Accurately implements treatment prescriptions including Sodium (Na) modeling prescription, and Ultrafiltration modeling (where appropriate) to ensure stable treatment therapy as indicated. Assesses patients’ response to hemodialysis treatment therapy, making appropriate adjustments and modifications to the treatment plan as indicated by the prescribing physician. Communicates problems or concerns to the Team Leader or physician. Identifies and communicates patient related issues to Team Leader or physician. Initiates Initial and Annual Nursing Assessment, and ongoing evaluation and documentation of patient care needs according to FMC Policies and Procedures. Actively participates in the pre-evaluation, initiation, monitoring, termination, access homeostasis, and post evaluation of patients receiving hemodialysis treatment therapy according to established FMC procedures. Takes appropriate intervention for changes in patient adequacy status and troubleshooting access flow issues as identified by OLC/AMP yellow lights. Provides, supervises (if applicable), and monitors hemodialysis access care according to established procedures. Implements, administers, monitors, and documents patient's response to prescribed interdialytic transfusions, including appropriate notification of adverse reactions to physician and appropriate blood supplier. Ensures accurate and complete documentation by Patient Care Technician on the Hemodialysis Treatment Sheet. Reviews, transcribes, and enters physician lab orders accurately into the Medical Information System. Ensures appropriate preparation of lab requisitions for Spectra or alternate lab. Ensures correct labs tubes are utilized for prescribed lab specimens and that lab draw and processing procedures are performed appropriately for all lab samples. Identifies and ensures appropriate follow-through regarding missed labs and specimens reported to be insufficient according to company policies and procedures. Ensures all specimens are appropriately packaged according to Department of Transportation (DOT) policies and procedures relating to shipment of blood or body fluid specimens and potentially hazardous material. The RN is responsible for assessing, planning, implementing, and evaluating nursing care provided to patients throughout the system. The RN must be highly knowledgeable and possess strong nursing skills necessary to provide excellent patient care that is appropriate to the ages of the patients served within their RN specialty. Registered Nurses at OSF HealthCare are licensed professionals empowered to participate in an interdisciplinary approach to meet the expectations of those we serve. Qualifications REQUIRED QUALIFICATIONS: Education: Associate’s Degree Experience: 2 years of nursing experience in an acute healthcare setting within the specialty hired, with at least one of those years in a telemetry, med surg or stepdown unit Licensure/ Certification: IL RN license Certified in AHA Healthcare Provider BLS within 6 months of hire Additional certifications as applicable to specialty Other skills/ knowledge: Must be willing and able to work in at least 3 OSF facilities as needed. Proven ability to demonstrate the knowledge and skills necessary to provide patient care appropriate to the age of the patients served on assigned units. Demonstrated knowledge of the principles of growth and development, as well as, the physical, emotional, and psycho-social needs of the patient population served. Strong interpersonal skills necessary to comfort patients, family members and others; to communicate effectively and concisely with other patient care providers; and to instruct patients, family members, and others in self-care and home care procedures. Excellent analytical ability necessary to assess patients' physical, psychological and social needs, develop individualized patient care plans, and evaluate patients' responses to medical and nursing interventions. Home Health RNs must have own transportation, auto insurance and abides by all laws, rules and recommendations for safe driving while on duty. Maintains a valid driver’s license.
