PCU Neuro Trauma is a 48 bed Neuro unit encompassing 8 EMU (epilepsy monitoring unit) beds. The Neuroscience Service Line at BUMCP offers state-of-the-art diagnostic and treatment modalities for patients while creating a rich educational and collaborative environment for nursing and medical staff. Our exciting accomplishments include laser thermal ablation of tumors, awake craniotomies, surgery for brainstem lesions, and endoscopic microsurgery. As the first comprehensive stroke center in Arizona, we take a multi-disciplinary team approach that involves nurses in the plan of care for the patient. Ratios flex between 1:3-4 and 1:6 in an RN/LPN team model. This opening is for full time day shift positions and will require 3 shifts per week from 7:00am-7:30pm with weekends and holidays included on a rotating basis. On call, weekends, holidays: rotating with other staff. POSITION SUMMARY This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. This position utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients. CORE FUNCTIONS 1. Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and assures completion within appropriate timeframe. Reviews available information obtained by other health care team members. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. Completes assessment and reassessments according to patient need and as outlined in policy. 2. Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. Goals for patient are realistic, measurable and developed in conjunction with the patient/family. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient’s status. 3. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided. 4. Evaluates the patient's and family's response to care and teaching, and revises the plan of care as needed. Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members. Evaluates patient's response and the effectiveness of patient teaching. 5. Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and in accordance with policy. Documentation reflects objective/subjective data, nursing interventions and patient’s response to treatment. Notes physician orders accurately and in a timely manner. 6. Provides care based on the best evidence available and may participate in research activities within clinical practice. Participates in unit or facility shared leadership. Interacts and participates in the education, role development, and orientation of facility personnel, patients, students, families and visitors. Promotes/supports growth of others through precepting and mentoring when appropriate. 7. Contributes to society through activities that lead to excellent patient outcomes through timely, effective, efficient, equitable, and safe care. PEDIATRIC HOSPICE RN CASE MANAGER JOB DESCRIPTION Overview The Hospice Pediatric RN Case Manager will work with our Pediatric team. Primarily responsible for providing professional nursing care to assigned patients. With input from the IDT (interdisciplinary team), patient and family, assesses, develops, implements and evaluates the plan of care. We place a high importance on our employees and reward staff in several ways such as: • Competitive hospice industry compensation • Benefits package with multiple plan offerings and generous employer contribution • 401(k) Retirement plan with employer match • Supportive work culture which encourages work life balance • Substantial Paid Time Off, paid holidays & birthday benefit • Employee development program managed by in-house education department to support individualized orientation and professional growth needs. • Monthly mileage reimbursement • Employee Referral Program • Business casual attire for office staff Responsibilities JOB DUTIES/KNOWLEDGE/JOB PERFORMANCE: • Develops written plan of care in coordination with an interdisciplinary Team (IDT). • Oversees implementation of plan, evaluates and changes plan as necessary to meet patient/family needs. • Responsible for the direction of care provided by LVN and HA in accordance with patient’s plan of care. • Responsible for the supervision of services provided by the LVN and HA per TEH policy and regulatory requirements. • Communicates all changes to IDT and primary MD. • Performs job duties in clinical areas as stated in clinical competency checklist, policies/procedures, standards of care, and critical competencies based on Scope of Practice. • Maintains timely patient documentation using “point care” electronic charting. • Implements and maintains established hospice policies and procedures as well as Standards of Care and regulatory requirements. • Provides health care instruction to patient and family members. • Orients, instructs, and trains assigned personnel. • Participates in providing 24 hour, seven days a week coverage to patients/families (on-call and census fluctuation). • Performs admission assessment and admission when necessary. DEGREE OF TRAVEL: • Travel required for patient visits. • Occasional travel for educational purposes. Qualifications: • Must have Associates Degree, Bachelor Degree preferred. • Must have one year of acute or hospice pediatric experience • At least 12 months experience with 1-2 years preferred in acute care. • Current CA RN license. • Current CA driver’s license with proof of insurance. • Current CPR/BLS • Prefer certification as hospice nurse. • Pediatric experience preferred. • Proof of recent TB test result • Bilingual preferred. • Computer skills (word processing), use of adding machine, calculator, fax, photocopier.
