With guidance and oversight from the DON, the CRM ensures completion of the assessment within the timeframe as mandated by federal regulations and company policies. The CRM also manages the overall process and tracking of all Medicare/Managed Care/Medicaid case mix documents in order to ensure appropriate and optimal reimbursement for services provided within the Care Center. Responsibilities also include assisting with the coordination of the resident care planning processes. Performs other duties as assigned. This is a safety sensitive position. Principal Accountabilities / Essential Job Functions: • Reviews the clinical records, MD progress notes, therapy and nursing documentation in order to capture all careand services for optimal reimbursement. • Ensures that participants in the assessment process complete an accurate and comprehensive assessment and collaborates with the Clinical Reimbursement Specialist (where applicable)or DON to address concerns. • Tracks Medicare/Managed Carebeneficiaries to determine continued and appropriate Medicare eligibility and benefit period by determining skilled level of need. • Performs concurrent and ongoing MDS review to ensure appropriate PDPM category is achieved through the capture of appropriate clinical information. • Manages the coordination of ICD-10 coding for Medicare and Managed Care billing. • Directs the interdisciplinary team process to communicate opportunities to ensure capturing of allcare, services, and diagnosis. • Coordinates with rehabilitation services Program Director, Corporate Director of Clinical Reimbursement and Central Billing Office as needed to communicate case mix data required foraccurate claim billing at month end. • Reviews additional document requests by local Medicare Administrative Contractors, insurance carriers or auditors to ensureappropriate documentation is submitted timely for review. • Encourages staff to report changes in the resident's status and involves the DON in addressing concerns. • Confirms that residents and their families are actively involved in the information sharing and decision-making process. • Completes resident assessment protocol documents within scope of practice. • Assists with the completion of the resident care plan and the care plan conferences per requirements as requested by the DON. • Provides resident and family education as needed. • Identifies and reports deviations from safe practice to the DON. Adheres to policies and guidelines of regulatory agencies (i.e. OSHA, CMS). • Manages emergency situations based on the Company’s safety and disaster policies. • Communicates and collaborates with other members of the healthcare team to resolve resident care problems and to enhance care delivery. • Participates in quality assessment/performance improvement activities and audits. • Maintains minimum data set competencies and attends annual educational programs. • Attends/participates in care center meetings, in-services and committee meetings. • May perform CPR, use Automated External Defibrillator (AED), and render first aid in emergency situations. A MSN or DNP are not required but supersede the requirement for the BSN. Purpose of Your Job Position The primary purpose of your position is to conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of this state and policies and goals of the Facility. Delegation of Authority As MDS Coordinator you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties. Conduct training programs for appropriate staff on the completion and use of the MDS. Assessment Function Ensure that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment functions and that they are aware of the penalties, including civil money penalties, for false certification. Ensure assessments accurately reflect the physical, mental and psychosocial status each resident. Ensure assessments reflect optimal reimbursement for services provided. Ensure appropriate documentation to support services provided. Initiates, directs and maintains the Medicare PPS and OBRA assessment schedules to ensure timely completion of all assessments. Complete entry trackers and discharge assessments accurately and timely. Ensure that an Initial Admission Assessment is completed within fourteen (14) days of the resident’s admission. Ensure that quarterly and annual resident assessments are completed on a timely basis. Collaborate with the IDC team to identify significant change in resident status and, ensures that a comprehensive resident assessment is scheduled within fourteen (14) days of a significant change in the resident’s condition. Ensure that each portion of the assessment is signed and dated by the person completing that portion of the MDS. Sign and date the assessment instrument to certify its completion. Ensure that all assessments are completed and transmitted in a timely manner. Review validation reports for fatal errors and warnings and completes Modifications and Inactivations in accordance with CMS Correction Policy. Maintain a PPS pathway for all residents on Medicare Part A and Managed Care. Collaborate with facility Director of Rehabilitation to ensure the most appropriate ARD is utilized for Medicare assessments thru the use of PPS pathways Provide billers with PPS pathways to ensure accurate billing. Complete Comprehensive Status Updates to Case Managers on all managed care residents per company guidelines. Complete and maintain timely completion and signing of Physician Medicare Certifications. Prepare for State and Federal agency audits related to RAI process and reimbursement. Resident Rights Review complaints and grievances made by the resident and make a written and oral report to the Nurse Supervisor, LPN, and RN. Follow Facility’s established procedures. Maintain a written record of the resident’s complaints and/or grievances that indicates the action taken to resolve the complaint and the current status of the complaint.
