You will attend discharge/multidisciplinary rounds in acute care, ambulatory and/or other settings within the health system to share your expertise and to assist the patient in transition of care from one setting to the next within the health system. We are seeking a dedicated Registered Nurse to join our Home Health team. to assume responsibility and accountability for the application of the nursing process and the delivery of patient care. The Registered Nurse (RN) demonstrates the ability to make clinical judgments in an effective and efficient manner under the direction of the Director of Clinical Services. Responsibilities Utilizes the nursing process to assess, plan, implement and evaluate patient care.
