SUMMARY Under general supervision, is responsible for providing medical treatment, first aid, emergency care and medications for students in the school programs, under the direction of a Registered Nurse and in accordance with company policies, the regulations and requirements of applicable regulatory agencies and the standards of the profession. ESSENTIAL DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Licensed Practical Nurse - Children's Services will include, but are not limited to the following: Serves and participates as a member of interdisciplinary team. Is responsible for assisting Registered Nurse in coordinating and following through on provisions of health care services for students in program. Keeps progress notes under supervision of registered nurse for each student and summarizes for quarterly, semi-annual and annual reviews. Provides regular and PRN health care to students receiving services as instructed by registered nurse. Provides direct hygiene and health instruction to staff, students receiving services and families as required. Handles control of communicable diseases and infections through identification and assessment. Reports to consulting physician and Infection Control Nurse. Implements appropriate protective and preventive measures. Provides staff training in regard to infection control regularly and as needed. Provides first aid to students in need of immediate care, with referral to registered nurse, private physician or outside emergency services as necessary. Monitors required staff health records and provides tests in accordance with regulatory agency requirements. Keeps up to date and accurate records of examinations, immunizations, illnesses, accidents and other pertinent data of students via progress notes. Administers medications as prescribed. Supervises secured storage of medications. Refers all consults and diagnostic reports to registered nurse for review. Transports students to appointments as necessary. Maintains Nursing Office. Orders supplies and equipment as required. Visits students at home as requested by Director or designee. Keeps abreast of changes in Nursing Services field. Performs other related duties as requested. QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 11. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. 4. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. The role of the registered nurse is to support the physicians and/or mid-level providers in the practice with clinical activities and direct patient care. The RN's focus in the cardiology practice will be phone triage and you must have attention to detail and ability to prioritize the acuity of each situation. Other responsibility may be to assist in appointment scheduling, nurse visits and other related activities for optimal patient service. RN's by nature of licensure have an oversight responsibility to other clinical staff (LPN and MA). Nurse RN - Flex Team | FT Days

Overview The Extended Flex RN provides nursing care to patients utilizing the nursing process and standards of practice for a professional nurse. The Extended Flex RN must comply with all license, certification, and training requirements required in those units. The Extended Flex RN is expected to perform all staff nurse activities and function as charge nurse when assigned. The Extended Flex RN maintains overall accountability/responsibility for assigned patients, even though some activities/tasks may be delegated to Technical Nurses, Patient Care Associates, and/or Sitters. Assignments are performed appropriately to age and developmental level of patients being served. The Extended Flex RN will be required to float to ICU/other areas holding ICU level patients, i.e. PACU, ED-ICU pod, Interventional Radiology, etc. The Extended Flex RN may also be required to operate in emergency response role as Rapid Response Team. The Extended Flex RN will be required to float to non-ICU or Specialty units based on staffing needs and conditions of the hospital. Qualifications Required Education: Associate's degree. Specific course work: Graduate of an accredited school of nursing. New hires effective June 14, 2017, must have a BSN degree from an accredited school of nursing within three (3) years of hire or transfer into the position. New hires or transfers into in the position between August 1, 2013 and June 13, 2017 must abide with the conditions in his or her offer letter and the verbiage in the job description signed upon hire or transfer into the position. Current employees who were hired or transferred into the position prior to August 1, 2013 will be grandfathered. Preferred Education: BSN Required Licensure/Certifications: Licensed to practice nursing in the State of Florida BLS from the American Heart Association ACLS from the American Heart Association Effective July 1, 2018, PALS from the American Heart Association is required within six (6) months of hire or transfer. Existing employees are required to have PALS by December 31, 2018. Professional Certification Preferred. Crisis Prevention Institute training within ninety (90) days, and then annually, is required for Psych Unit.
