Schedule 5 days a week 37.5 - 40 hours per week Respiratory Therapist (RT) / Respiratory Nurse (RN) We are searching for a highly motivated, flexible, organized, skilled respiratory therapist who has experience in a rehab center and possesses great communication skills to put the “therapy” into respiratory therapy. As a Post Acute and Long Term Care respiratory therapist, your role would involve providing specialized care and treatment to patients with cardio-pulmonary, respiratory conditions neurological disorders and physical conditions that manifest into respiratory disorders over an extended period. Your primary responsibility would be to assess, provide diagnostic testing, develop treatment plans, and provide treatment to our Skilled Nursing Facility population. Job Description ESSENTIAL JOB FUNCTIONS: 1. Provides patient treatments which meet patient needs and uses current treatment knowledge in accordance with facilities clinical procedures. This includes but is not limited to: · Performing accurate, comprehensive patient evaluations. · Developing an effective plan of treatment and obtaining appropriate approvals from a referring physician. · Communicating with supervisor and interdisciplinary team members regarding patient progress, problems, and treatment plans. · Instructing patient’s families and/or nursing staff in specified follow through programs. · Interviewing patients and families regarding previous level of functioning, lifestyle and current and future expectations for recovery. · Demonstrating knowledge of appropriate resources for problem-solving and clinical best practice. · Evaluating patients within the required timeframe as per facility policies. · Provides patient care as appropriate and necessary, following the position description for the applicable clinical discipline in which licensure or certification is held. 2. Writes accurate, complete, and clear documentation in accordance with the facility’s regulatory, licensing, payor, and accrediting requirements. This includes but is not limited to: · Recording Screens/Resident Needs Reviews, evaluations, daily treatment notes, discharge summaries and walking rounds in accordance with facility Policies and Procedures. · Recording treatment charges accurately and ensuring that documentation is consistent with billing data. · Writing specific, objective documentation that is outcome oriented and details the patient’s needs, potential to benefit from treatment, and progress achieved to date. Includes specific, objective measurable goals. 3 . Consistently demonstrates facility interdisciplinary approach to patient care. This includes but is not limited to: Integrating findings of other members of the interdisciplinary team in treatment assessment and recommendations. Integrating input from patient and family in treatment assessment and recommendations. Preparing for and providing meaningful contributions to the rehab team conferences, patient care conferences, utilization review meetings, family conferences, and caregiver training sessions. Collaborating with and informing facility staff in areas of patient progress. Educating members of the team in areas of expertise. Working with a facility to teach documentation practices which assist the respiratory therapy team. Communicating effectively with discharge planners. 4. Contributes to professional development of self. · Maintaining current professional status and licensure. · Adhering to professional standards of conduct and clinical code of ethics and practice. · Implementing in-services and training. · Maintaining a current body of knowledge in areas of clinical expertise through seminars, professional journals, and peers. · Meeting continuing education requirements of regulatory agencies. 5. Aids in building and maintaining the facility’s reputation as the unparalleled respiratory therapy provider by contribution as a professional member of the staff. · This includes but is not limited to oversight of the following departmental operations: · Consistently demonstrating the facility’s core values by interacting with co-workers, patients and families, with all other persons involved with the patient’s care. · Consistently demonstrating professional standards as outlined in the facility’s Employee Handbook and as communicated by the facility’s management. · Contributing to a positive work team by sharing information, using problem solving methods and accepting new ideas, constructive criticism, or advice from others. · Maintaining close communications within the facility with the director of nursing, nurse managers, and admissions coordinator to assist in development of the respiratory therapy program. · Establishing and maintaining an effective rapport with the facility administrator, MDS, Rehab Director, DON, and medical director. o Demonstrating superior customer service through courtesy, consistent follow-up and positive communication. 6. Demonstrates safe working practices . This includes but is not limited to: Identifying and correcting safety hazards or notifying the facility or the admin so that safety hazards will be immediately remedied. Maintaining work areas in safe and orderly fashion. Maintaining equipment in safe working order by facilitation annual calibration. Using proper body mechanics during treatments of patients and during support activities. Using universal safety precautions. Wearing appropriate protective equipment. Overview Cooper University’s Neonatal Intensive Care Unit is a 35-bed level 3 nursery. This unit is set up in smaller pods which hold up to 4 infants, except for Transitional nursery which holds up to 12 infants. These infants are often preparing for discharge. We care for infants as young as 23 weeks gestation as well as infants requiring surgical interventions, respiratory support from nasal cannula up to and including high frequency ventilation and nitric oxide. Cooper’s NICU has a Neonatal transport team that will go out and pick up infants requiring a higher level of care, from different hospitals throughout the South Jersey area. Our NICU consists of all Registered RN’s, attending physicians, fellows, Neonatal Nurse Practitioners as well as Residents. We also have a robust ancillary team including dedicated Respiratory Therapists, Occupational Therapists, Physical Therapists, Speech Therapists that address all the feeding issues of the individual infants. PURPOSE AND SCOPE: The Inpatient RN provides direct patient care in accordance to company policies and procedures which includes FMCNA compliance programs and contracted hospitals’ policies and procedures. Areas of practice are diverse, including, but not limited to, hemodialysis, peritoneal dialysis, continuous renal replacement therapies, apheresis, and ultrafiltration. The Inpatient RN will partner with the hospital to provide quality care to the patient and deliver care that provides a positive patient experience. PRINCIPAL DUTIES AND RESPONSIBILITIES: Under limited supervision, applies considerable knowledge to perform a broad range of tasks which include but are not limited to direct patient care in multiple modalities not limited to hemodialysis, peritoneal dialysis, ultrafiltration, continuous renal replacement therapy, and Apheresis. Provides supervision and direction of Patient Care Technicians and clerical staff; provides direction and education to ICU staff for CRRT. Responsible for direct patient care of assigned patients in the Inpatient Renal Replacement and Apheresis Services setting and providing the appropriate treatment technique for assigned patients as prescribed in all modality procedures, as appropriately defined – hemodialysis, peritoneal, continuous renal replacement therapies, ultrafiltration, and Apheresis. Assesses patients’ responses to treatment therapy making appropriate adjustments and modifications to the treatment plan as indicated by the appropriately credentialed physician. Communicate problems or concerns to the Inpatient Program Manager, appropriately credentialed physician, patient hospital primary nurse and others as indicated. Delivers safe, effective care in a timely an efficient manner. Gives organized, concise shift reports for patients on assigned shifts. Apply high level thinking skills to shift patient schedules to meet needs of more critical patients due to labs, MD order, or patient assessment. Responsible for the implementation, administration, monitoring, and documentation of patient’s response to prescribed intradialytic transfusions, including appropriate notification of adverse reactions to physician and appropriate blood supplier. Collaborates and communicates with physicians and other members of the healthcare team to interpret, adjust, and coordinate daily patient care plan to ensure continuity of care. Initiates and communicates to FMS facility nursing staff and appropriate hospital personnel as needed. Accurately documents all treatment information in the individual patient record (ACES) and the hospital record, as needed. Cleans and disinfects dialysis machine surface, chair, equipment, and surrounding areas between treatments according to inpatient renal services policies and procedures. Conducts all tasks necessary for preparation for dialysis treatment and documents where appropriate. Performs all required pre-treatment dialysis machine alarm testing. If applicable, initiates Solution Delivery System. Operates all related equipment appropriately and safely and provides minor trouble shooting when necessary; efficiently utilizes supplies to prevent waste. Understands, conducts, and documents appropriately Dialysis/Apheresis machine safety tests/alarm tests, equipment calibration, dialysate testing, machine safety tests, functional testing, and internal and external disinfection on all water & dialysis machines, and complies with the documentation/notification standards per FMS policies. Completes and documents ongoing participation in QAI activities. Participates in staff meetings as scheduled. Participates in staff training and orientation as assigned. Acts as a subject matter expert to hospital staff for dialysis related patient/equipment complications. Responsible for reporting all new or unusual incidents, information, complaints, or problems to the inpatient manager and other parties per the Adverse Event Report requirements.
