Home Infusion Registered Nurse (RN)

Enliven is seeking an experienced RN (Registered Nurse) to service home infusion and Infusion Center patients in Tyler and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patients' well-being by providing high-quality nursing care. Responsibilities: RN (Registered Nurse) Complete daily electronic forms for each assigned patient within a secure Electronic Medical Record (EMR) System. Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients. Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager. Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation. Report patient care, conditions, and progress to the patient's physician, pharmacy, and Clinical Manager continuously. Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems. Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation. Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care. What You'll Do Direct and oversee all aspects of the Nursing Department. Ensure the delivery of high-quality, resident-centered care. Lead, coach, and develop nursing managers and staff. Maintain compliance with all federal, state, and local regulations. Monitor clinical programs, quality measures, and infection prevention initiatives. Partner with the interdisciplinary team to improve resident outcomes and satisfaction. Support recruitment, retention, and staff engagement efforts. Oversee department budgets and effective resource utilization. Lead survey preparation efforts and manage regulatory inspections. Implement initiatives that promote operational excellence and continuous improvement. What We're Looking For Current and active Registered Nurse (RN) license in New York. Minimum of three years of nursing leadership experience in a Skilled Nursing Facility or Long-Term Care setting. Previous experience as a Director of Nursing, Assistant Director of Nursing, or senior nursing leader preferred. Strong knowledge of CMS regulations, survey processes, and quality improvement initiatives. Excellent leadership, communication, and organizational skills.
