Responsibilities PRIMARY Ability to provide quality care in both clinic and telehealth and meet patient volume goals targeted for tele and in patient as determined. Ability to meet patient volume goals targeted for tele and in-patient as determined. Ensure accurate completion of patient charts in a timely matter and forwarding charts as appropriate on a daily basis. As well as build and maintain confidence and credibility with all employees. Implement clinical and Telehealth protocols as outlined CMO and Supervising Physicians. Analyze and interpret patients' histories, symptoms, physical findings, and diagnostic information to develop appropriate diagnoses. Deliver excellent patient care through in-patient and virtual diagnostic and therapeutic recommendations with attention to patient-centric care, safety, cost, and reliably accurate information. Ensure that the activities of the Provider are conducted in a manner that is consistent with overall department expectations and are in compliance with Federal and State regulations, guidelines, and requirements including working knowledge of all health information management issues such as HIPAA. Ability to work efficiently in a fast-paced, autonomous environment within both in-patient clinic and virtual settings. Ability to pursue queue of telehealth patients in a timely fashion to ensure proper patient follow up. Order, perform, or interpret the results of diagnostic tests, as well as responsibly prescribe medications and educate patients on continued treatment and care of acute and chronic conditions. Attest and follow clinical practice guidelines by the Office of Medical Affairs. Attend Mandatory monthly meetings with Supervising physicians and E/M training. Ability to perform responsibilities within standard NP protocols. Job Description UCSF Benioff Children's Hospital is recognized throughout the world as a leader in health care, known for innovation, technology and compassionate care. Our 183-bed UCSF Benioff Children's Hospital has more than 150 specialists in more than 40 areas of medicine. The Pediatric Intensive Care Unit (PICU) at UCSF Benioff Children's Hospital provides extraordinary high-level care for children who require continuous monitoring. The unit has 20 beds for infants and children suffering from a variety of conditions or recovering from surgery. More than 1,000 children per year are admitted into the intensive care units at UCSF Benioff Children's Hospital arriving via ambulance, helicopter and airplane from throughout the western United States. The Pediatric Transitional Care Unit (TCUP) is a 18-bed unit for infants and children suffering from a variety of conditions or recovering from surgery. The TCU provides care to patients with a moderate to high complexity of disease such as those recovering from solid organ transplantation, titration of anticoagulant therapy and other infusion, and post-operative specialty surgery care (ENT, neurosurgery). Patients requiring respiratory support including BIPAP, CPAP, high flow nasal cannula and patients requiring mechanical ventilation with tracheostomy in place are also routinely admitted to the TCUP. The Nurse Practitioner will act as a full member of the team taking responsibility for the safe delivery of care in collaboration with attending physicians, fellows and residents. This team member will meet productivity expectations as set forth by the Division Chief, performing any duties related to patient care activities that are appropriate and clearly delineated. Nurse Practitioners on the PICU and TCUP Team provide care in rotating 12h shifts. Responsibilities Function % of time Duties Inpatient - Direct Patient Care (may include some or all of the responsibilities listed) 70 Independently (independent APP encounter) or collaboratively (shared physician and APP encounter) evaluates and manages patients. Includes taking histories, performing physical examinations, responding to changes in patient status, performing consults, ordering or furnishing medications/diagnostic tests/treatments/therapies/consultations/referrals/durable medical equipment (DME)/home health services. Care provided must be documented in the electronic medical record in accordance with the UCSF Rules and Regulations . Provider services must be billed to reflect the performing provider and comply with the APP Billing Policy 3.08.01 . APPs must respond to clinical documentation integrity inquiries from the Department of Quality and Patient Safety within 3 days. 1.2. Performs procedures granted to them via privileges/standardized procedures. 1.3. Identifies patients with acute decompensation or failure and responds to the hospital plan of care and reviews with physician. 1.4. Initiates consultations as indicated. 1.5. Documents independent assessment findings and recommends appropriate treatments. 1.6. Participates in service rounds and patient evaluation and treatment planning. 1.7. Reviews lab tests, x-rays, and other diagnostic tests, and initiates appropriate team plans. 1.8. Evaluates complex cases, establishes individualized plans of care for disposition to a lower level of care. 1.9. Interprets and acts upon completed tests, procedures, and radiographic studies. 1.10. Performs procedures individually granted to them via standardized procedures/privileges. 1.3. Identifies high-risk groups and complex cases for review with attending physician. Evaluates complex cases, establishes individualized plans of care for rehabilitation, and facilitates occupational and physical therapy and physical medicine and rehabilitation consultations. 1.5. Interprets and acts upon the ordered tests, procedures, and radiographic studies in a timely fashion. 1.6. Completes clinically relevant paperwork/EHR, including but not limited to ordering medications, completing episodic care notes and telephone encounters. Outpatient – Shared Encounters with Physician(s) (may include some or all of the responsibilities listed) ## Evaluates and manages patients in shared encounters with attending physician, including telehealth. This should increase clinic access with a goal of doubling clinic volume since two providers will be sharing the clinic responsibilities. Includes providing care to new, follow-up, acute, routine, and specialty patients. Includes taking histories, performing physical examinations, interpreting diagnostic tests, collaborating on evidence-based assessments and plans, responding to changes in patient status, and ordering medications/diagnostic tests/treatments/therapies/consultations/referrals/durable medical equipment (DME)/home health services. This APP position may perform delegated functions as appropriate to their “scope of practice”. In general, the “scope of practice” includes the management of chronic stable medical conditions, specialty protocols (e.g. chemotherapy), routine care, acute or critical care, and medical and surgical procedures. This position will provide outpatient care in approved age groups at UCSF Health settings. The APP has responsibility for the quality of standards of care for a specified patient population and evidences self-direction and accountability in the development of this role within the all UCSF medical center locations. The APRN is responsible for clinical oversight and health center consultation regarding administration of health care policy and procedure of the delivery of on-site health services. The NP promotes optimum health, prevents illness and injury, and manages acute and chronic health problems in the on-site setting. Performs physical examinations and preventive health measures within prescribed guidelines and instructions of Physician. Orders, interprets, and evaluates diagnostic tests to identify and assess patients clinical problems and health care needs. Job Title/Position: Nurse Practitioner Reports To: Clinical Director JOB DESCRIPTION SUMMARY The Nurse Practitioner is a registered professional nurse with advanced graduate education and clinical training who provides comprehensive hospice services in compliance with the state Nurse Practice Act and under the supervision of a collaborating physician. He/She provides hospice care through the evaluation and assessment of the health history, health promotion, diagnosing medical conditions, management of health problems by directing and developing the plan of care, prescribing medications or treatments, coordinating and collaborating with patients, families and other healthcare practitioners. Under the supervision of a physician, the nurse practitioner visits patients, completes physical assessments, performs face to face assessments and evaluates hospice eligibility as assigned in consultation with the hospice interdisciplinary team. The nurse practitioner builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Provides medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician. 2. Completes history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record. 3. Manages health conditions and terminal illnesses by developing an individualized plan of care, prescribing medications and / or treatments, obtaining consultations or making referrals and coordinating hospice care services. 4. Completes the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations. Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification. 5. Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care. 6. Reviews clinical information, coordinates and oversees patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners. 7. Assures that patients receive appropriate measures to control symptoms, through collaboration with interdisciplinary team members. 8. Reviews and develops protocols for treatment and proposes options for interventions based on the literature in collaboration with the collaborating physician. 9. Consults the collaborating physician when the patient's plan of care is outside standardized practice and protocols. Communication 1. Communicates and collaborates with the interdisciplinary group to create, review and revise the patient's plan of care. 2. Consults with the patient's attending physician, the hospice medical director and other healthcare practitioners regarding the patient's eligibility for hospice care, ongoing care needs and medical management related to the patient's terminal condition. 3. Communicates with other community health practitioners to coordinate the plan of care. 4. Attends and participates in hospice interdisciplinary group meetings. 5. Completes, maintains and submits accurate and relevant clinical notes, physical examination notes, assessment visit notes, medical orders, collaboration notes and other documentation in the medical record. 6. Educates/counsels patients, families, and/or caregivers as to preventative care, medical problems, psychological problems, and spiritual problems in conjunction with the interdisciplinary team to meet the total needs of patients. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to identify the goals of care and meet the care needs of the patient and family/caregiver. 10. Serves as a nursing resource for consultation and education to members of the interdisciplinary team and other healthcare practitioners in the community. 11. Establishes, builds and nurtures relationships with staff and community referral sources to facilitate program growth. Additional Duties 1. Maintains knowledge of and compliance with current Medicare/Medicaid, state/federal rules and regulations for hospice services 2. Ensures compliance with the Medicare conditions of participation and other state regulations govern the provision of healthcare. 3. Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state and organizational policies. 4. Participates in organizational monitoring of the quality of medical services and quality improvement initiatives. 5. Assumes responsibility for personal growth. Develops, maintains and upgrades professional knowledge and practice skills through attendance at seminars, conferences and participation in continuing education and in-service classes. 6. Fulfills the obligation of requested and/or accepted assignments. 7. Demonstrates knowledge in communication and counseling patients/families in dealing with end-of-life issues. POSITION QUALIFICATIONS 1. Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program. 2. Has a current DEA license, if applicable to state regulations 3. Current nursing licensure in State and CPR certification. 4. Master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience. 5. Accreditation from an approved certifying body for advanced practice nursing as required by State. 6. Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics) 7. Excellent observation, verbal and written communication skill, problem solving skills, mathematical skills; nursing skills per competency checklist. 8. Prolonged or considerable walking or standing. Able to lift, position and / or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills. 9. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
