Position Summary The Registered Nurse Navigator coordinates and manages the care needs of patients with urogynecologic conditions, providing personalized support, education, and advocacy from diagnosis through treatment and follow-up care. Balance clinical needs with resource utilization and financial considerations to support efficient and effective care delivery. Qualifications: Requires an Associates of Nursing (ASN). Bachelors of Nursing (BSN) preferred. Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license. Requires that the RN has graduated from a nationally accredited nursing program. 3-5 years of experience preferred. Requires Basic Life Support certification through the AHA. Other advanced life support certifications may be required per unit/department specialty according to patient care policies. Specialty certifications may be required per unit/department specialty in accordance with the department provision of care. General Role Description: The Home Health Patient Care Manager/RN is responsible for the supervision and coordination of clinical services and provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
