Assess data related to basic, physical, developmental, spiritual, cultural, functional, and psychosocial needs of the patient. Collects data within established protocols and guidelines from various sources. Documents data collection, assessment, and communicates findings to appropriate healthcare members. Care plan development and implementation with health care team members collaborating to develop long and short term patient goals. Communicates with Physician using Situation, Background, Assessment and Recommendation format. Contributes to the interdisciplinary team to manage Wound Care patients. Patient documentation is accurate and timely and it reflects rationale for any deviation from standards of care. Practice is congruent with evidence based wound care guidelines. Observing Universal Protocol at all times provides clinical assistance to patients and notifies appropriate medical personnel of potential critical patient conditions.
