Job Summary GENERAL SUMMARY We are seeking a highly skilled, compassionate Registered Nurse (RN) to deliver high-quality, patient-centered care through a combination of in-home patient visits, telehealth patient consults with Nurse Practitioners or Physicians as required, and administrative tasks. The CareConnect Team at WellBe supports timely response to patients’ changing needs and this role will be instrumental in delivering this care consistently and in a patient-first manner. This role primarily supports urgent/episodic interventions, working in partnership with physicians, nurse practitioners (APPs), and interdisciplinary care teams to address a range of patient needs across the care continuum. Previous experience in hospice, palliative care, or geriatrics strongly preferred; other complex patient health care experience will be considered.
