Description The Assistant Nurse Manager is a Registered Professional Nurse with broad knowledge and skills who acts as clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Clinical Nurse Manager. Responsibilities Patient Care 1. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. 2. Job Description The Stroke Navigator assists the Stroke Medical Director and Stroke Program Director to improve the quality/delivery of ischemic and hemorrhagic stroke care throughout the continuum of care. He/she works in collaboration with the program directors, data analysts, department leaders, staff and patients to develop and support the stroke program’s performance improvement initiatives. Prepares nursing daily and monthly schedule within operating grids and budget. Monitors department on an ongoing basis and adjusts FTE to patient census via staffing grid and nursing protocols. Directs staff rotations and reschedules personnel to meet increased or decreased nursing service demands. Completes timecard corrections and reports any issues with tardiness, missed lunches or missed punches to the DON. Oversees attendance, missed punches and lunch breaks through monitoring and corrective action when needed. Complete any audits regarding staffing, including the annual staffing evaluation. Teach the patient, family, and other members of the health care team. Perform services in accordance with the physician’s orders and the established plan of care (POC). Qualifications Must be a graduate of an approved school of clinical education. Must be licensed in the state where they currently practice. Must have at least one year experience as a licensed professional. Must have basic demonstrated technology skills, including operation of a mobile device. Education and experience, preferred Experience as a licensed professional may be deferred with a review and approval from the regional vice president or regional director. Medicare home health or hospice experience is preferred.
